What is a Census?
A census provides basic info such as:
- Number of full-time employees
- Employee name
- Birthday
- Gender
- Dependent Name, Birthdate, and gender for those also wanting insurance
- Zip code of Residence
Once collected, the insurance company takes the data and determines the premium rates based on the information provided.
What is a Health Questionnaire?
Sometimes an employer would like to provide a health plan that requires medical information to be provided to the insurance company. Depending on the insurance company requirements, a health questionnaire may be required and it asks for more information from the employee. The health questionnaire paints a well-rounded picture of the employee’s health.
The questionnaire may ask for:
- Medical history for the last 5 years
- Any recent doctor visits
- Any recent ER / hospital stays / surgeries
- Medications, including dosage and frequency
If a health questionnaire is required, the information should be kept confidential and submitted directly by the employee to Advanced Benefit Solutions. It should not be given to the employer.
Health questionnaires also ask for recent hospitalization and doctor visits so they can factor in any ongoing conditions. Large premium increases are often caused by expensive medical conditions from the previous year that weren’t taken into account at the time of setting the insurance premium.
Therefore, if an insurance broker doesn’t request a health questionnaire when one is required, there is no way for them to know and account for that data in their initial rate calculation.
